1. No purchase is necessary to win and as an entrant you have not yet won. Entries into the Revolver/Mayhem Festival Sweepstakes must be received no later than 11:59 p.m. PT on August 19, 2008 (the "Deadline"). The winners will be chosen on or about August 25, 2008 and will be notified by e-mail (or regular mail). The odds of winning depend upon the number of entries received.
2. To enter by mail, print your name, address, mobile number, e-mail address and "Revolver/Mayhem Festival Contest" on a 3 x 5 card and mail it to: Revolver/Mayhem Festival Contest C/O MARKETING, 149 5th Ave., New York, NY 10010. Only one entry will be accepted.
3. 4. Winners will be determined by a single random drawing of all valid entries received by the Deadline and the decision of Future US shall be final. You may obtain the name of the winner by sending a stamped, self-addressed envelope to the address above. Future US reserves the right, in its sole discretion, to cancel or suspend this drawing should viruses, bugs, Internet failure or other causes beyond its control corrupt the administration, security, or proper play of the drawing.
5. All entrants who complete the registration form will be considered as participants.
6. Entrants must be at least 13 years of age. If a winner is a minor, the prize will be awarded in the name of winner's parent or legal guardian, who is solely responsible for all applicable federal, state and local taxes, and that winner must also provide a signed parental or guardian consent form, provided by the sponsor, within 7 days of notification attempt. Failure of the minor to return the guardian consent form will result in the forfeiture of the prize and selection of an alternate winner, who will be chosen through a random drawing from the remaining eligible entries.
7. Return of any prize or prize notification as undeliverable will result in disqualification and an alternate winner will be selected.
8. The winners may be required to sign an affidavit of eligibility/release of liability/prize acceptance within seven days of receipt; failure to do so upon request will result in forfeiture of the prize. By acceptance of the prize, the winners agree to the use of their name and/or likeness for purposes of advertising, trade, or promotion without further compensation, unless prohibited by law. Winners are responsible for paying any income taxes on the value of the prize received.
9. By entering the Sweepstakes, you (and, if you are a minor, your parents or legal guardians) release Future US, Mayhem Festival, EMG, SNAPJACK, MUSICIANS INSTITUTE, and any of their respective parent companies, subsidiaries, affiliates, directors, officers, employees and agencies (collectively, the "Released Parties") from any liability whatsoever, and waive any and all causes of action, related to any claims, costs, injuries, losses, or damages of any kind arising out of or in connection with the Promotion or delivery, misdelivery, acceptance, possession, use of or inability to use any prize (including, without limitation, claims, costs, injuries, losses and damages related to personal injuries, death, damage to or destruction of property, rights of publicity or privacy, defamation or portrayal in a false light, whether intentional or unintentional), whether under a theory of contract, tort (including negligence), warranty or other theory.
10. This Sweepstakes is open to residents of the United States only. Void in Vermont, Rhode Island, Puerto Rico and where prohibited by law.
11. Employees, officers, and directors of Future US, Inc., the publisher of their subsidiaries and affiliated companies, and their immediate families and those living in their households, are not eligible.
